The Adult Care Facilities are categorized by type of care provided: Adult Care Homes
(assisted living – small and larger), Adult Day Care (8:00-5:00) and Skilled Nursing
Facilities. View a complete

- Adult Care Homes (fewer than 6 beds) - These small residential facilities
provide care at the assisted living level. Forsyth County DSS provides monitoring
and facility complaint investigation services for these homes.
- Adult Care Homes (more than 6 beds) - These larger residential facilities
provide care at the assisted living level. Forsyth County DSS provides monitoring
and facility complaint investigation services for these homes.
- Adult Day Care Centers - These centers provide care and supervision for adults
during the day. Forsyth County DSS provides certification, monitoring and complaint
investigation services for these centers.
- Skilled Nursing Facilities - These residential facilities provide skilled
nursing services. Additionally, some provide a combination of skilled and intermediate
nursing care as well as assisted living level of care. The NC Division of Facility
Services license, monitor and provide complaint investigation services for these
facilities.
Placement
Social Worker/Adult Home Specialists, assist individuals and their family members
with identifying appropriate placement within the community which includes adult
care facilities.
Medicaid Enhanced Personal Care
Social Worker/Adult Home Specialists provides case management services for residents
in need of Medicaid Enhanced Personal Care Services in the adult care facilities.
The overall provision of care and/or services that are provided must adequately
meet the residents’ needs and are consistent with the existing standards for quality
care. The case manager verifies Medicaid eligibility and the need for Enhanced Personal
Care Services and authorizes the additional hours of PCS in the areas of toileting,
eating and ambulation/locomotion.
Licensing/Monitoring
Article 1, Chapter 108A of the North Carolina General Statutes requires that County
Departments of Social Services foster a protective environment for frail and vulnerable
adults served by the adult care facilities’ industry. The Social Worker/Adult Home
Specialists assure that the various types of adult care facilities operate within
the applicable rules, regulations and laws through on-going planning and the licensure/monitoring
process.
Adult Care facilities that are monitored range in size from family care homes with
as little as two adults to larger homes with as many as one hundred adults. The
Adult Home Specialists also investigate facility complaints. Major areas monitored
are: Residents’ Rights, Staffing, Food Service, Drug Management and Health Care.
