Obtaining a Certified Copy
|
We may only issue copies of records that are filed in Forsyth County.
Certified copies of these records may be obtained by appearing in person at the
Register of Deeds or by mail. Certified copies have a raised seal and are good for
any legal purpose. We cannot record birth, death or marriages from other counties,
states or countries.
The cost per copy in North Carolina is $10.00 (No personal checks accepted). For
a recent death or birth, please allow 10 days to 2 weeks for us to receive the certificate.

Who Can Obtain Certified Copies
|
You can obtain your own certificate. A spouse, brother, sister, child, parent, authorized
agent, attorney or legal representative may obtain it for you.

Requesting Birth Certificates by Mail
|
Send a written request with the person's full name at birth, date of birth, place
of birth, the father's name, the mother's maiden name, and your relationship to
the individual. Print an
Application for Certified Copy of a Vital Record on which to make your request.

Requesting Death Certificates by Mail
|
Send a written request with the deceased person's full name, date of death, place
of death, the father's name, the mother's maiden name, and your relationship to
the individual. Print an
Application for Certified Copy of a Vital Record on which to make your request.

Requesting Copies of Marriage Certificates by Mail
|
Send the groom's full name, the bride's name before the marriage and the place and
date of the marriage. Print an
Application for Certified Copy of a Vital Record on which to make your request.
PERSONAL CHECKS ARE NOT ACCEPTED. DO NOT SEND CASH THROUGH THE MAIL. A money order
should be made payable to the Forsyth County Register of Deeds. A self-addressed
envelope will ensure prompt return. We try to answer written requests within two
days of receipt. Mail your request to:
