The Cleveland Avenue Dental Center is part of the Forsyth County Department of Public Health. We serve children and adults of Forsyth County and surrounding areas. We accept Medicaid, NC Health Choice, Ameritas and self-pay patients. A sliding fee scale may be applied for self-pay patients based on household income.
We are a fully functioning dental center for both children and adults. Our caring, compassionate staff will help ensure that you have a pleasant dental experience.

The dental center is located on the corner of N Cleveland Ave & 5th street. We are directly across from Bethlehem Community Child Development Center on North Cleveland Avenue.

Loretta graduated from Forsyth Tech Community College in 2007. Loretta has been a dental assistant with Forsyth County Public Health for the past 8 years. She enjoys helping patients achieve a beautiful and confident smile.
Lauren has 9 years of dental assisting experience. Lauren loves encouraging patients through dental treatment. Lauren is knowledgeable in lab cases that include dentures and partials; Lauren enjoys every patient leaving with a smile.
McKenzie graduated from NC Dental U located in Wake Forest in 2014. McKenzie has been a Dental Assistant for almost 3 years. Since braces, she has always had a passion for teeth. She loves to spend time outside with her dog, enjoys riding her horses, and enjoys riding four wheelers on the weekends. She likes to be the first person you see when entering our office, and the last person you see before you leave!
Kia is a native of Winston Salem. Kia has over 20 years of experience in the dental field, as a regional trainer and dental assistant. She has a passion for serving the community, through educating adults and children on dental hygiene through her non-profit work in Forsyth County. Outside of work Kia enjoys spending time with her close-knit family.
Cancellation Policy: 24 working hours notice is required for all cancelled appointments. If a patient fails to call with advance notice, the appointment will count as a broken appointment. After 2 broken appointments, the patient will be dismissed from the practice.
Late Arrival Policy: Please call if you are going to be late for your appointment. If you are more than 10 minutes late for your appointment you may be rescheduled for a later date.
Co-pay: A $3 co-pay will be required of NC Medicaid and NC Health Choice patients, age 21 and over, at the time of service.
Parent/legal guardian must be present for the 1st visit.
