The Cleveland Avenue Dental Center is part of the Forsyth County Department of Public Health. We serve children and adults of Forsyth County and surrounding areas. We accept Medicaid, NC Health Choice, Ameritas and self-pay patients. A sliding fee scale may be applied for self-pay patients based on household income.
We are a fully functioning dental center for both children and adults. Our caring, compassionate staff will help ensure that you have a pleasant dental experience.

The dental center is located on the corner of N Cleveland Ave & 5th street. We are directly across from Bethlehem Community Child Development Center on North Cleveland Avenue.

Loretta graduated from Forsyth Tech Community College in 2007. Loretta has been a dental assistant with Forsyth County Public Health for the past 8 years. She enjoys helping patients achieve a beautiful and confident smile.
Jenni graduated from the UNC School of Dentistry Dental Assisting Program in May 2017. Jenni is very passionate about her work. She is one of our bilingual staff members. Jenni is empathetic and hardworking. She strongly believes that everyone deserves access to quality dental care. Outside of work, she enjoys spending time with her husband, running, and cooking.
Lauren has 9 years of dental assisting experience. Lauren loves encouraging patients through dental treatment. Lauren is knowledgeable in lab cases that include dentures and partials; Lauren enjoys every patient leaving with a smile.
Patricia received her dental assistant certificate in 2009 from Wilkes Community College. She fell in love with dentistry and decided to pursue her career in hygiene. Patricia graduated from Forsyth Technical Community College as a Dental Hygienist. She has participated in many volunteer service projects with the MOM's clinic and enjoys working with adults and children of all ages.
Madison graduated from High Point University in 2013 with a Bachelor of Science in Biology. Madison always wanted to enter the field of Dentistry, so she decided to pursue her Associate of Applied Science in Dental Hygiene and graduated from the program in May 2017. She enjoys interacting with her patients on a personal and professional level. At home, she enjoys spending time with her husband and beloved pets.
Cancellation Policy: 24 working hours notice is required for all cancelled appointments. If a patient fails to call with advance notice, the appointment will count as a broken appointment. After 2 broken appointments, the patient will be dismissed from the practice.
Late Arrival Policy: Please call if you are going to be late for your appointment. If you are more than 10 minutes late for your appointment you may be rescheduled for a later date.
Co-pay: A $3 co-pay will be required of NC Medicaid and NC Health Choice patients, age 21 and over, at the time of service.
Parent/legal guardian must be present for the 1st visit.
