CC4C Managing Toxic Stress

Did you Know that “CC4C” “Care Coordination For Children”, help families who have children between the ages of 0-5 that are considered Toxic stress situations? Toxic Stress can be defined as an individual who experiences strong, frequent, and or prolonged adversity such as physical or emotional abuse, chronic neglect, caregiver substance abuse or mental illness, exposure to violence, and or the accumulated burdens of family economic hardship, without adequate support.

In tackling the adversity in Forsyth County, the CC4C program has established partnerships with DSS, Salvation Army, local hospitals, therapists, doctors, and many more to gain referrals and to help create avenues, resources and hope to those not sure how to create an outlet with their current toxic situation.

Many NICU babies for example, may need assistance with cognitive and developmental growth once discharged from the hospital, a care manager and guardian if interested would refer the patient to the Children’s Development Service Agency commonly known as (CDSA) . CC4C also helps with resiliency when looking thru the scope of foster care. Many children placed in foster care may need assistance with speech referrals, behavioral specialists or even counseling to help cope with the new change.

CC4C creates that link with primary care providers to help the transition feasible for the parent and child. There are many other unique variables that care managers encounter when referencing toxic stress. In igniting the key components of toxic stress with the CC4C program it allows care managers to create goals with families and providers while allowing the child to gain many opportunities for health, growth and success . If ever a child is in need, indeed try referring that child to the “CC4C” Program, in efforts to promote a healthy outcome, for a better tomorrow.
